Enrollment Requirements

Thank you for your interest in Old Mission School. To be considered for enrollment, please complete and submit an application online. The following items need to be submitted with the online application.

  • Completed new student application
  • $67.00 non-refundable application fee
  • Copy of birth certificate
  • Copy of Catholic baptismal certificate (if applicable)
  • Copy of First Communion certificate (if applicable)
  • Copies of two most recent years report cards to date (if applicable)
  • Copies of two most recent years standardized test scores (if applicable)


Our goal is to provide the best learning environment for all students. Failure to disclose learning and/or behavioral help needs may jeopardize acceptance into our school.

Upon completion of paperwork:

  • For TK: “Meet & Greet” with teacher scheduled and completed.
  • Grades K-8: Family interview scheduled and completed. Kindergarten readiness screening completed. Math placement test completed for incoming 7th grade students. 

 

Upon acceptance to Old Mission School the following must be submitted to complete our school records:

Registration Fees:

New Students $375.00 Returning Students $300 by March 1

  • Completion of Admission Contract
  • Report of a health examination completed within 12 months of enrollment (form completed by physician).
  • Copy of up-to-date immunizations according to state law
    • TK & Kindergarten: 4 Polio, 5 DTaP, 3 Hep B, 2 MMR, 2 Varicella
    • Grade 7: Tdap booster
